Irregular Menstrual Cycle Treatment That Starts with the Cause

For the full picture of what is normal, read our periods and menstrual health guide.
What Counts as an Irregular Period?
Your cycle length is the number of days from the start of one period to the start of the next. For most adult women that is somewhere between 24 and 38 days, not exactly 28.
It is called irregular when it keeps falling outside that range or you can no longer predict it. Uneven cycles are expected for a few years after periods begin and again before menopause.
Signs Your Cycle Is Irregular
- Periods less than 24 days or more than 38 days apart
- Cycle length that shifts by more than about nine days
- Three months or more without a period, and not pregnant
- Bleeding or spotting in between periods
- A period that goes on for more than eight days
Personalized Care for a Healthy Menstrual Cycle
An irregular cycle is a symptom, not a diagnosis. Dr. Grishma Ranjangaonkar (MBBS, DGO), a gynecologist with 15+ years of experience, starts with your period history and advises only the tests likely to explain the pattern.
What follows depends on the findings and on what you want: predictable cycles, lighter bleeding, contraception or a pregnancy. Some women need only reassurance, many need medicines for a while, and a few need a procedure. When polycystic ovaries are the cause, see our page on PCOD treatment in Vashi.
Common Causes of Irregular Periods
More than one cause can be present, so the pattern of your bleeding matters as much as any single test.
Hormonal Conditions
PCOD is one of the commonest reasons for periods that are far apart or missing. Thyroid disorders and a raised prolactin level can upset the cycle in much the same way.
Life Stage
Cycles are often uneven soon after the first period, after childbirth and in perimenopause. Starting or stopping a contraceptive method can also alter bleeding for a few months.
Weight, Stress and Other Causes
Ongoing stress, rapid weight loss or gain and very intense exercise can delay or stop ovulation. Fibroids, polyps and certain medicines more often cause heavy periods or bleeding between periods.
Our Irregular Menstrual Cycle Treatment Services
Types of Treatments
- Hormonal treatment: tablets that bring on regular, lighter bleeds, chosen to suit your age and health.
- Ovulation support: medicines that help the ovary release an egg, if you are trying to conceive.
- Lifestyle measures: changes to food, activity and sleep when weight or routine is part of the cause.
Diagnostic Process
- History and examination: your period dates, symptoms and medicines, with a physical check where needed.
- Tests you may be advised: a pregnancy test and blood tests for hemoglobin, thyroid and prolactin. Other hormone tests only when your pattern points to them.
- Pelvic ultrasound: checks the ovaries and uterine lining for polycystic ovaries, fibroids or polyps.
Treatment for Underlying Conditions
- PCOD or PCOS: cycle regulation alongside weight and lifestyle advice.
- Thyroid or prolactin problems: treated with medicines, with a physician involved when that is needed.
- Fibroids, polyps and endometriosis: medicines first where suitable, and keyhole surgery only if it is likely to help.
Support & Counseling
- Fertility assessment: a check on ovulation if you are planning a pregnancy.
- Time to talk: room to raise worries about missed periods, weight or fertility without being hurried.
- Follow-up: review visits to see how your cycle responds and adjust the plan.
When Irregular Bleeding Needs Prompt Attention
Most irregular periods can wait for a planned appointment. A few patterns should be seen quickly, and very heavy bleeding, fainting or severe pain needs a hospital emergency department, not a clinic slot.
If you are unsure how soon to be seen, our symptom checker sorts symptoms by urgency, though it cannot diagnose.
Do Not Wait If You Have
- Bleeding that soaks through a pad every hour for several hours
- Dizziness, fainting or breathlessness along with heavy bleeding
- A missed period with severe pain low on one side
- Any bleeding in pregnancy or after a positive pregnancy test
- Bleeding after menopause, a year or more since your last period
- Bleeding after sex that keeps coming back

Frequently Asked Questions (FAQs)
What causes irregular menstrual cycles?
PCOD (also called PCOS) and thyroid disorders are among the commonest medical causes. Stress, a marked change in weight, breastfeeding and the approach of menopause can disturb the cycle too, so a check-up is the reliable way to learn which applies to you.
How do I know if I have an irregular cycle?
Write down the first day of each period for three or four months. Gaps shorter than 24 days or longer than 38 days, or lengths that swing widely from month to month, count as irregular. A single early or late period is common and rarely a concern.
Can I get pregnant if my periods are irregular?
Yes, many women with irregular cycles do conceive. Irregular periods often mean you ovulate less often or unpredictably, so it can take longer. Treating the cause, and sometimes medicines to support ovulation, may help, although results vary. If you do not want a pregnancy, you still need contraception.
How long does it take to regulate my cycle?
It depends on the cause and the treatment chosen. Hormonal tablets usually give a predictable bleed within the first few cycles, while changes that come from weight, thyroid treatment or lifestyle often take several months to show.
Is the treatment for irregular cycles painful?
Treatment for an irregular menstrual cycle is mostly tablets, lifestyle changes and follow-up, so it is generally not painful. An internal ultrasound, if advised, can feel uncomfortable briefly, and an abdominal scan is normally used instead for women who are not sexually active.
Do irregular periods always need treatment?
No. If no worrying cause is found, the bleeding is not heavy and you are not trying to conceive, reassurance and a review may be enough. Treatment is usually advised when periods are heavy, when you want a pregnancy, or when PCOD leaves you without a period for three months or more, because the uterine lining can thicken over time.
Are irregular periods normal in teenagers and before menopause?
Often, yes. Cycles commonly take two to three years to settle after the first period and become uneven again before menopause. A check-up is still sensible if a teenager goes more than three months without a period, or if bleeding in your forties becomes very heavy, very frequent or appears between periods.
Will I have to take hormone tablets to regulate my periods?
Not necessarily. Hormone tablets are one option among several, and they control bleeding while you take them without curing the underlying cause. They do not suit everyone, so your doctor will ask about migraine, blood pressure, smoking and any history of clots before prescribing.
